Eligibility Requirements:

• Referred by a physician

• Approved by local Community Long-Term Care (CLTC)

• Self-referrals

• Approved by Disability Special Needs Board (DSNB)

Veterans or spouses of veterans in need of in home care may qualify for benefits through the Veteran's Administration.
